Community Health Center of Yavapai

The Community Health Center of Yavapai is a Federally Qualified Health Center committed to providing a medical home for patients through the delivery of primary care services. The services are provided to people who have no medical insurance, or are enrolled in Medicaid, Medicare or Blue Cross/Blue Shield. Uninsured patient fees are based on a sliding fee scale according to income. Community Health Center of Yavapai has five locations throughout Yavapai County, Prescott Valley, Prescott, Chino Valley, Cottonwood, and Sedona. "The mission of the Community Health Center is to contribute to the well being of the people of Yavapai County by offering quality integrated primary health services".
