Bomb Threat Guidance: This document provides guidance on responding to bomb threats and includes a call-taker checklist to document essential information from the person making the threat. (added March 31, 2014)

Security Checklist: This document provides a comprehensive checklist pertaining to security at the facility.  It is recommended that this form be downloaded and added to the facility Emergency Operations Plan (disaster plan) upon completion. (added March 31, 2014)

Hazard Vulnerability Assessment Tool: This spreadsheet tool gives the health center the ability to assess the probability of a wide variety of specific types of hazards and perils and gauge levels of preparedness.  This tool provides the facility with a Risk Analysis of both internal and external adverse events that could potentially impact operations. Proximal Hazards Chart: This chart is an accompaniment to the facility Hazards Vulnerability Assessment (HVA) to further identify potential hazards and perils that may be in close proximity to the clinic and identify specific threats to operations. (added March 31, 2014)
