  • Units of J1055 and total purchased inventory of Depo Provera
  • Last 6 months charges, payments, adjustments and visits related to these charges (if you can delineate this by major payers (e.g. AHCCCS, Medicare, BCBS, commercial, and self pay)…all the better).  DON'T FORGET DENTAL!!
  • Total AR with AND without Self Pay
  • Units of 99201 – 99205 and 99211 -99215

Bring any reports you love, don't understand or those on which you want other opinions.
