You are invited to this FREE annual symposium sponsored by the Arizona Department of Health Services, Bureau of Tobacco & Chronic Disease, in partnership with Arizona Alliance for Community Heath Center, Heath Services Advisory Group, American Heart Association and University of Arizona Medication Management Center.

WHEN: Friday, December 1, 2017.  Registration and networking start at 7:30am. Symposium starts at 8:30am and will end at 2:00pm.

WHERE: Desert Willow Conference Center, 4340 E Cotton Center Blvd, Phoenix, AZ 85040

TARGET AUDIENCE:  Pharmacists and Staff of Community Health Centers, Rural Health Clinics and Physician Group Practices (Administrators, CQI Directors and Managers, HIT professionals, Providers, and others).

SYMPOSIUM GOALS:

  • Educate primary care clinic staff and pharmacists on the benefits and critical components of Clinical Quality Improvement (CQI) programs.
  • Provide participants with the knowledge and tools necessary to implement a successful CQI program.
  • Highlight local best practices in clinical quality improvement and collaborative practice agreements related to chronic disease.
