We are pleased to share an exciting opportunity at Terros Health for a COMMUNITY HEALTH WORKER SUPERVISOR.

The Community Health Worker Supervisor provides day-to-day supervision of Community Health Worker (CHW) team and guides them to make a profound impact on the lives of underserved individuals and families living with complex and/or chronic medical and behavioral conditions. The CHW team improves patient access to local resources and healthcare, with the goal of improving their wellness and quality of life. The Supervisor helps address social determinants of health (conditions in the environments in which people are born, live, learn, work, play, etc. that affect a wide range of health outcomes and risks) and ensures CHWs assist community members with health specific education, navigation and access to community services and other resources, and self-managing health conditions and adopting improved health behaviors. This position coordinates daily operation of all activities and records the activities and progress of programs.  In addition, this position ensures accurate collection of data to generate reports and billing in a timely manner.  This position coordinates with Director of Integrated Operations  to ensure that staff are provided with the necessary resources and supplies to meet all contractual obligations.  The Supervisor also assists with ongoing program development, management, and training.

Main Duties:

Community Health Worker Supervisor Duties Include:

  • Supervise Community Health Workers (CHWs) daily activities: Provide intensive and ongoing supervision to the CHW team as they engage a targeted patient/community member population around health issues.
  • Ongoing evaluation of a team of CHWs.
  • Support CHWs: Follow up (via phone or in person) with CHWs on a regular basis with 1:1 meetings, coaching sessions, directing CHWs resources, developing support materials, identifying necessary CHW training needs, developing program materials/tools to support CHWs in their job.
  • Ensure the CHWs track patients/community members and report progress on activities and services, this includes conducting regular monitoring assessments on data entry status, developing quality improvement protocols, and conducting data quality data checks.
  • Serves patients in a similar manner as Community Health Workers. Includes assisting clients in their homes, community, or clinic setting, documenting client encounters, maintaining client files, and documenting activities. Educates clients, assists them in accessing health related and social services, provides support and advocacy and facilitates access to community resources.
  • Facilitate closing gaps in care by assisting CHWs to educate patients about preventive monitoring and working with providers and patients to schedule diagnostic testing.

Education - BA degree in public health or related field. At least one year of supervisory experience. Life/work experience working in the field of integrated health care field.


  • Excellent written and verbal communication skills.
  • Proficient in Microsoft Office. NextGen experience highly valued.
  • Ability to drive throughout Maricopa County.
  • Flexibility and ability to work evenings and weekends.
  • Ability to work with all cultures and sub-cultures that comprise the population of Maricopa County in a non-judgmental and value clear manner.
  • Promotes, models and uses strength-based engagement and treatment strategies for all patients served.  Lends their unique insight into behavioral and/or physical health recovery.
  • Excellent interpersonal skills both in person and by phone, with high professionalism.
  • Must have valid Arizona driver’s license, be 22 years of age with minimum 3 years driving experience and meet requirements of Terros Health’s driving policy.
  • Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role. (HR to assist in this process and fingerprinting.)
  • Lived experience in behavioral health or medical area, and ability to obtain certification within 90 days of employment
  • Must pass a TB and Drug Screen Test
