Community Health Center patients often require assistance with determining their eligibility for state, federal, or community programs. Complex paperwork, language barriers, and other requirements often hinder patients from getting the help they need.

AACHC provides outreach and education activities for Outreach and Enrollment/Eligibility Staff, providing training and technical assistance to these vital advocates in Community Health Centers. Community Health Center Eligibility Staff provide outreach and application assistance for various programs, including:

  • TANF (cash assistance)
  • Public housing
  • SNAP (formerly food stamps)
  • AHCCCS (Medicaid in Arizona)
  • Sliding Fee Scale (no insurance, pay based on what you earn)
  • Referral to experts who help with Medicare questions and enrollment

Community Health Center Outreach and Enrollment staff are trained:

  • To provide accurate information, competent enrollment assistance, and follow-up assistance for various health coverage and benefit programs.
  • To refer clients to appropriate community resources for additional services, including housing, food, and other assistance programs.

AACHC supports its member Community Health Centers in their outreach efforts by:

  • Helping Community Health Center staff remain current and excited about their outreach work.
  • Facilitating communications and information sharing, conference calls, site visits, and meetings.
  • Facilitating and conducting Health-e-Arizona trainings and Medicare trainings.
